Health History Requirement

Before coming to Cornell, all new students must submit a Health History Form to verify that they have met their health requirements, including documentation of required immunizations

Cornell Health, Cornell's on-campus health center, will use this information to comply with state and federal law, and to provide students with the best possible care while studying at Cornell.

Health Services

Cornell Health provides medical care, counseling, pharmacy services, and other resources to support the well-being of all students studying on Cornell's Ithaca campus. Services are confidential, and designed to meet the needs of Cornell's diverse student population.

Tips & Topics

  • myCornellHealth: Cornell Health's secure patient portal

Students can use myCornellHealth (NetID required to login) to schedule or cancel appointments, communicate and share confidential information with your health care provider, ask a nurse a health-related question, access health records and billing statements, and more.

  • Student Disability Services

Cornell's Student Disability Services (SDS) office—part of Cornell Health—helps students with physical, medical, mental health, and sensory disabilities participate fully in the Cornell experience. If you have a disability for which you need accommodations, please register your needs directly with SDS

  • Resources for parents and guardians

Through surveys and anecdotes, Cornell students make it clear that their parents/guardians are their most important source of information and support related to their health and well-being. Cornell Health provides online resources for parents and guardians, especially for those whose sons and daughters are just beginning their Cornell experience.
